ACR Contrast Allergy Premedication Protocol for Imaging Centers: Guide Published

Oct 23, 2026

New guidance from ContrastConnect explains how the 2026 ACR contrast allergy recommendations impact premedication decisions, contrast-agent selection, and patient safety, with virtual radiologist supervision supporting imaging centers’ coverage needs.

Las Vegas, United States, October 23, 2026 /PressCable/ -- ContrastConnect has published a guide explaining the 2026 ACR contrast allergy premedication guidelines and their implications for imaging centers. The resource covers updates to steroid premedication, contrast-agent substitution based on reaction severity, and requirements for radiologist supervision during contrast administration.

More details can be found at https://www.contrast-connect.com/blog-post/acr-contrast-allergy-premedication-protocol-2026-guidelines-explained.

The new recommendations follow a 2025 joint consensus statement from the American College of Radiology (ACR) and the American Academy of Allergy, Asthma & Immunology (AAAAI). For patients with prior mild immediate reactions to iodinated contrast, routine steroid premedication is no longer recommended. Instead, the guidance supports substituting the documented inciting agent, making accurate records of reactions and contrast products essential for clinical decisions.

According to the ContrastConnect guide, premedication may be considered alongside agent substitution for moderate reactions. For patients with severe reaction histories, the guidance recommends considering imaging without iodinated contrast first. If no suitable alternative exists, premedication and switching agents when feasible may be appropriate.

When premedication is indicated for scheduled exams, the standard oral regimen includes prednisone at 13, 7, and 1 hour before contrast, plus diphenhydramine one hour before the procedure. Urgent cases may use an accelerated five-hour IV hydrocortisone regimen with diphenhydramine. A Radiology study found breakthrough reaction rates of 2.5% with the accelerated regimen versus 2.1% with the 13-hour oral protocol, meeting noninferiority criteria. Regimens shorter than four to five hours lack supporting evidence unless a life-threatening situation makes delay impossible.

The guide also reviews risk factors, including prior contrast reactions, asthma, and significant atopy. Shellfish allergy and topical iodine exposure are not independent risk factors. Documenting the specific contrast agent in the electronic health record helps teams assess substitution options.

Implementing these recommendations also requires imaging centers to consider radiologist availability during contrast administration. Effective January 1, 2026, the Centers for Medicare & Medicaid Services (CMS) permanently recognizes real-time, interactive audio-video presence as meeting direct supervision requirements in applicable outpatient settings, where permitted by state law.

To address radiologist coverage needs, ContrastConnect offers virtual contrast supervision through licensed radiologists, with immediate availability and documentation for CMS review. The company reports supervising more than 75,000 hours of contrast exams and managing over 130 contrast reactions monthly, offering imaging centers an alternative to onsite radiologist staffing.
